Job summary

You will assist with medication administration, meal preparation, transportation, and documentation, while collaborating with an interdisciplinary team and mentors to promote independence and engagement in daily routines.

Qualifications

• Must have ability to communicate and understand written and oral directives.
• Support individuals to participate in daily activities while ensuring safety.
• Physical aspects may require lifting or transferring individuals as needed.

Responsibilities

• Provides ongoing guidance and support to individuals to ensure active participation in daily life.
• Assist individuals with self-care, health, safety, and household chores as needed.
• Help individuals become active in the community and maintain relationships.
• Follow treatment plans and use safety equipment to promote independence.
• Assist with meal preparation and nutrition concepts.
• Assist with medication administration according to policy.
• Document treatment plans and progress in agency records.
• Adhere to staffing policies and participate in team meetings.
• Provide transportation for medical appointments and activities.
• Mentor new Direct Support Staff and support training.
• Participate in house meetings and complete required certifications.
• Perform other duties as requested by supervisor.
