Job summary

AHRC Nassau is seeking a Direct Support Professional to provide ongoing guidance and support to individuals enrolled in Community Living Services, aiming to maintain a high quality of life, health, safety and wellbeing.

You will assist with self-care routines, meals, medication administration, and everyday tasks, while promoting independence and community participation within a supportive team environment.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license and maintain driver status.
• Ability to understand written and oral directives.

Responsibilities

• Provide ongoing support to individuals to enable independence in daily routines.
• Assist with self-care, health, safety, and household chores.
• Support medication administration per AMAP policy.
• Transport individuals to medical appointments and activities.
• Attend inter-disciplinary meetings and participate in goal progress discussions.
• Mentor new Direct Support Staff and review training materials.
