Job summary

AHRC Nassau in New York state seeks Direct Support Professionals to assist individuals with intellectual and other disabilities in achieving independence and meaningful daily activities. The role focuses on developing self-care skills, safety, meal preparation, and community participation, while following treatment plans and documenting progress.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license and maintain qualified driver status.
• Ability to communicate and understand written and oral directives.

Responsibilities

• Provide ongoing guidance and support to individuals to promote independence and engagement.
• Assist individuals to become active in the community and maintain relationships.
• Keep homes safe and clean; help with household chores as needed.
• Follow treatment plans and use adaptive equipment for health and safety.
• Assist with meal preparation and dietary guidance.
• Assist with medication administration according to policy.
• Document treatment plans and notes to track progress.
• Attend meetings and participate in interdisciplinary team discussions.
• Provide transportation for medical appointments, shopping, and outings.
• Mentor new Direct Support Staff and support training.
