Job summary

AHRC Nassau is seeking a Direct Support Professional to assist individuals with intellectual and other disabilities in daily living, health, safety, and community participation. You will help with self-care, meal preparation, medication administration, and transportation to appointments and activities.

The role requires adherence to plans, documentation, and collaboration with the inter-disciplinary team to support independence and well-being.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver’s license and maintain qualified driver status throughout employment.
• Ability to communicate and understand written and oral directives/information.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life.
• Supports individuals in self-care routines, health, safety, household chores, socialization and wellbeing.
• Assists individuals with medication administration according to agency policy and procedures.
• Implements and documents treatment plans and other agency materials to promote independence and wellbeing.
• Provides transportation to medical appointments, shopping, recreational outings, and other needs.
• Performs duties as requested by supervisor.
• Participates in meetings to discuss progress and goals.
