Job summary

AHRC Nassau is seeking a Direct Support Professional to assist individuals with intellectual and other disabilities in Community Living Services. You will support self-care, health, safety, and daily living activities to promote independence and a high quality of life.

The role emphasizes teamwork, documentation, and using adaptive equipment as needed. Responsibilities include assisting with meals, transportation, community participation, and adherence to treatment plans.

Qualifications

• High School Diploma or equivalent preferred.
• Must have NYS driver's license and maintain qualified driver status.
• Ability to communicate and understand written and oral directives.

Responsibilities

• Provides ongoing guidance and support to individuals to ensure active participation in daily living activities.
• Provide only the level of support necessary for independence and engagement in routines.
• Supports individuals in becoming active community members and maintains natural supports.
• Keeps home safe and clean by assisting with household chores and participating as needed.
• Follows treatment plans and uses adaptive equipment to promote health, safety and well-being.
• Assists with meal preparation including diet adherence and portion control.
• Assists individuals with medication administration according to policy.
• Documents treatment plans, goals and notes to aid independence and well-being.
• Participates in meetings, mentors new staff, and adheres to scheduling policies.
• Provides transportation to medical appointments, shopping and outings.
