Center for Disability Services, Inc. is seeking a Direct Support Professional II for Residential Services in the Capital Region of New York. The role supports individuals with disabilities through planning, care delivery, and daily living activities, in a 24/7 residential setting.

• One year of related experience preferred.

Responsibilities

• Serve as a member of the person’s planning, implementation, and interdisciplinary teams.
• Assist, promote and provide independence in daily treatment, care and services; respect choices, lifestyle and dignity to take informed risks.
• Help implement personalized habilitation services as outlined in the care plan.
• Assist in acquisition of skills including basic activities of daily living, self-care and household management.

Rate of pay is commensurate on experience, Residential Program acuity, shift differentials and completion of additional trainings.
