OneWell Health Care seeks a dedicated Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will assist with daily living activities, promote independence, and help with health and safety monitoring while adhering to care plans and regulatory standards.

The role emphasizes mobility, meal support, documentation, and collaboration with an interdisciplinary team to ensure dignity and inclusion in community life.

Qualifications

• High school diploma or equivalent.
• Preferred: caregiving, home health, or direct support experience with IDD.
• Experience in community-based or residential support settings is a plus.
• English proficiency required; bilingual skills are a plus depending on program needs.

Responsibilities

• Assist with daily living activities and personal care.
• Support meal preparation, nutrition, mobility, and medication administration as authorized.
• Implement behavior support strategies and monitor health and safety.
• Promote independence and community participation through structured routines and transportation support.
• Maintain documentation and participate in trainings and team meetings.
