Job summary

OneWell Health Care seeks a dedicated Direct Support Professional (DSP) to provide person-centered support for individuals with IDD and Autism in Connecticut. The DSP will assist with daily living activities, implement service plans, monitor health and safety, and promote independence in community and home settings.

The role involves documentation, collaboration with interdisciplinary teams, and adherence to regulatory standards.

Qualifications

• High School Diploma or GED required
• Minimum age 18 years
• English proficiency required (spoken and written)

Responsibilities

• Provide person-centered support to individuals with IDD and Autism.
• Assist with activities of daily living (ADLs) and skill development.
• Implement ISPs and BSPs, monitor health and safety, and document accurately and timely.
• Collaborate with interdisciplinary teams and participate in required trainings and meetings.
