Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide person-centered care to individuals with IDD in home and community settings. The DSP promotes independence, safety, and community participation while following ISP/IPP guidance.

Responsibilities include assisting with daily living, meal prep, mobility, documentation, and behavior support under supervision. Strong communication and confidentiality are essential; field-based travel may be involved.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving or home health preferred.
• Ability to use electronic documentation and EVV systems.
• English proficiency required; bilingual is a plus.

Responsibilities

• Provide hands-on support with daily living activities.
• Assist with meal prep, mobility, and medication as authorized.
• Maintain care plans, document notes, and report changes.
• Support community participation and transportation as needed.
