Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community-based settings. You will assist with daily living activities, health and safety monitoring, and community participation under the guidance of the interdisciplinary team.

The role emphasizes adherence to individualized service plans, regular documentation, and flexibility in scheduling, including evenings and weekends.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support preferred.
• Ability to maintain HIPAA-compliant documentation and confidentiality.
• Proficiency with electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered support per care plans.
• Assist with daily living activities, nutrition, mobility, and medication assistance as authorized.
• Document accurately in required timeframes and maintain records.
• Support transportation to appointments and community activities and participate in trainings.
