Job summary

Qualifications

• HS diploma required as minimum educational qualification.
• Experience working with individuals with mental health diagnoses or developmental disabilities or substance use disorders, depending on population served.

Responsibilities

• Assess individuals to understand hopes, dreams, and desires and provide supports.
• Help people develop relationships in their community with natural supports.
• Assist participation in community activities aligned with interests.
• Protect and promote rights; support advocacy skills and involvement in advocacy.
• Provide emotional, physical, and medical support as needed.
• Foster person-centered, positive relationships promoting self-determination.
• Document goals, actions, outcomes, incidents, timesheets, and related records.
• Assist with daily living tasks and transportation as required.
• Ensure safety procedures and emergency knowledge are followed and drillings conducted.
• Support new staff with orientation and training; participate in meetings and trainings.
