Qualifications

• High School Diploma is required.
• Valid driver's license in the USA is required.
• Experience with individuals with mental health diagnoses or developmental disabilities or substance use disorders preferred.

Responsibilities

• Assess people’s hopes and goals and provide supports.
• Help individuals develop community relationships and natural supports.
• Assist with daily living activities and transportation as needed.
• Ensure rights protection and advocate for the people served.
• Complete documentation, progress notes, and logs accurately.
