This PRN, on-site position requires compassionate care, strong communication, and commitment to person-centered planning within Monarch's LTSS program.

Qualifications

• High School Diploma required.

Responsibilities

• Assess people for hopes, dreams, and desires and provide supports to help achieve them.
• Help people develop relationships in their community and with natural supports.
• Assist people in participating fully in their community aligned with their interests.
• Ensure rights of people receiving services are protected and promoted at all times.
• Assist with advocacy skills, advocacy efforts, and use of personal advocates.
