Qualifications

Responsibilities

• Support health and wellness, including assisting with medications as directed.
• Assist with mobility, transfers, and accessibility in home and community settings.
• Document daily notes, progress and incidents accurately in electronic systems.
• Support community engagement, advocacy, and participation in programs.
