Job summary

WellLife Network is hiring a Direct Support Professional to assist individuals with daily living activities, health needs, and community participation in Brooklyn, NY.

You will provide compassionate hands-on care, help with meals, groom daily routines, and support medical appointments while advocating for rights and independence. This role emphasizes teamwork, documentation, and person-centered care.

Qualifications

• High School diploma or equivalency diploma required.
• Experience working with adults with intellectual/developmental or other behavioral health disabilities preferred.

Responsibilities

• Provide hands-on care, encouragement, and advocacy for individuals.
• Assist with daily living including bathing, dressing, grooming, and toileting.
• Help with meal preparation and household chores; maintain a safe home environment.
• Administer medications as directed and support medical appointment attendance.
• Assist with walking, transfers, and use of mobility aids; ensure safe navigation.
• Support behaviors using respectful strategies and provide encouragement.
• Plan and participate in recreational, educational, and social activities; foster community involvement.
• Assist with communication tools to help individuals express themselves; advocate for needs and access to services.
• Collaborate with healthcare providers, therapists, and other team members; maintain accurate daily notes and reports.
