Job summary

WellLife Network is seeking a Direct Support Professional to provide compassionate, person-centered care for adults with developmental or behavioral health needs in a home and community setting.

You will assist with daily living activities, coordinate with healthcare teams, and help individuals participate in meaningful community experiences while documenting progress in electronic systems.

Qualifications

• High School diploma or equivalent required.
• Experience working with adults with intellectual/developmental or other behavioral health disabilities preferred.
• Ability to communicate effectively with individuals served, families, and teams.

Responsibilities

• Provide hands-on personal care and daily living support with dignity.
• Administer medications as directed and accompany medical appointments.
• Assist mobility, transfers, and use of mobility aids safely.
• Implement behavioral strategies with respect and encouragement.
• Plan and participate in community, educational, and social activities.
• Support communication and advocate for needs and rights.
• Document daily notes, progress, and reports in electronic systems.
