Oak Hill in Burlington, CT is seeking Direct Support Professionals for a per-diem substitute role with a minimum 15 hours per month. Compensation starts at $20.39/hour with shift-based differentials.

The position includes training, background checks, and opportunities to support daily living activities, health and wellness, and community inclusion. The role requires transporting individuals, documenting health information in the EHR, and following safety protocols.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment per Oak Hill policies.
• Commitment to continuity of care, including occasional stay beyond scheduled shift.

Responsabilités

• Assisting with personal care, grooming, bathing, dressing, and hygiene needs.
• Monitoring health information and documenting as required.
• Transporting individuals to appointments and activities using agency vehicles.
• Assisting with meals according to menus and dietary needs.
• Helping with mobility and transfers using approved techniques and equipment.
• Documenting daily activities and medications in the electronic health record (EHR).
• Responding to emergencies and applying First Aid/CPR as needed.
