Job summary

Oak Hill in West Hartford, CT is seeking a Direct Support Professional to help individuals lead healthy, meaningful lives. The role is 30 hours weekly, Monday–Friday, 9:00 AM–3:00 PM, with a base pay of $20.39/hour and differential pay for select schedules.

You will assist with daily living, health monitoring, medications, nutrition, mobility, documentation in the EHR, and transportation to events and appointments. Paid training is provided.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver’s license to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and health screenings.
• Ability to complete required training programs and maintain certifications.
• Commitment to a safe, healthy, professional work environment per Oak Hill policies.
• Commitment to continuity of care, including occasionally staying beyond scheduled shifts.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene as per individual plans.
• Provide continence care as needed and encourage independence.
• Monitor health information and document essential data; administer medications per directives.
• Follow health care plans and support overall well‑being.
• Transport individuals to appointments and activities; support community participation.
• Prepare meals and provide mealtime assistance according to dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and incidents in the EHR; comply with regulations.
