Job summary

Oak Hill is seeking a Direct Support Professional in Waterbury, CT. This union, non-exempt position offers 40 hours per week with Sunday–Thursday 2 PM–10 PM, and a base pay of $20.39 per hour with differential pay for specific schedules.

You will support individuals with daily living activities, health and wellness, community inclusion, nutrition, and mobility. Documentation is required in the EHR and adherence to health care plans and regulations.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.
• Preferred: Previous experience as a Direct Support Professional (DSP), CNA, caregiver, home health aide, behavioral health technician, or related role.
• Preferred: Experience supporting individuals with intellectual or developmental disabilities.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene per support plans.
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor health information and vital signs as required.
• Administer medications and maintain medication records per directives.
• Follow health care plans and support overall well-being.
• Transport individuals to appointments and community activities using agency vehicles.
• Encourage participation in community life and personal interests.
• Prepare meals and snacks per menus and dietary needs.
• Assist mobility and transfers with approved equipment.
• Document daily activities and health information in the EHR.
• Follow all regulatory requirements (Oak Hill, DDS, DPH, HIPAA).
• Respond to emergencies and apply First Aid/CPR when required.
• Communicate effectively with colleagues, individuals served, families, and other stakeholders.
• Participate in meetings, trainings, and ongoing professional development.
