Job summary

Hope-Haven-Inc. is seeking Direct Support Professionals (DSP) to support clients with intellectual disabilities or chronic mental health conditions, helping them meet physical, emotional, and social needs. You will work with individuals to implement personalized plans and promote independence.

Duties include daily living assistance, documentation of logs, transportation coordination, and safety adherence. Requires a high school diploma or GED, good communication, a valid driver’s license, and

Qualifications

• High school diploma or GED/HSED preferred.
• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment in making decisions.
• Ability to complete/pass required medication administration course.
• Valid Class C driver’s license required.
• Clearance from applicable background checks as required by service standards.
• Reliable transportation.
• Proof of state minimum liability insurance coverage.

Responsibilities

• Provide support based on individualized service plans to meet physical, mental, emotional, and social needs.
• Assist with daily living activities including hygiene, bathing, toileting, escort, mealtime, and transition times.
• Teach self-control and self-management per training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Support individuals in developing daily schedules of activities.
• Arrange appointments and transportation as needed with proper documentation.
• Promote personal safety and follow all safety procedures.
