• Develop individualized goals with HCBS Supervisor and Programmer.
• Provide direct care, promote independence, and support community participation.
• Maintain accurate Setworks documentation for all interventions daily.
• Assist with daily living activities, transportation, and safety monitoring.
• Administer medications per policy and ensure scheduling and training requirements are met.
