Job summary

Hope Haven Inc. is seeking Direct Support Professionals (DSP) to assist clients with intellectual disabilities or chronic mental health conditions. The role focuses on helping clients achieve personal goals while ensuring physical, mental, emotional, and social needs are met.

You will provide training, assist with daily activities, document client information, and coordinate schedules and transportation. A valid driver’s license, good judgment, and strong communication are essential.

Qualifications

• High School Diploma or GED/HSED preferred.
• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment in decisions.
• Ability to complete/pass required medication administration course.
• Valid Class C driver’s license required.
• Clear background checks as required by standards.
• Reliable transportation.
• Provide proof of state minimum liability insurance.

Responsibilities

• Support individuals based on goals and service plans, ensuring physical, mental, emotional and social needsare met.
• Provide support and training, including hygiene, mobility, and daily living activities as needed.
• Teach self-control and self-management according to training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Support clients in daily activity schedules tailored to their needs.
• Assist with appointments and transportation and ensure documentation is complete.
• Promote safety and follow all safety procedures.
