Job summary

Hope Haven Inc. is seeking Direct Support Professionals to assist clients with intellectual disabilities or chronic mental health conditions. You will help meet physical, emotional, and social needs and support individuals according to their personalized goals.

Responsibilities include implementing the service plan, assisting with daily activities, documentation, and arranging appointments. A valid driver’s license and background checks are required; opportunities for comprehensive benefits are

Qualifications

• High school diploma or GED/HGED is preferred.
• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment in making decisions.
• Ability to complete/pass required medication administration course.
• Valid Class C driver’s license required.
• Clearance from all background checks as required by service standards.
• Reliable transportation.
• Proof of state minimum liability insurance coverage.

Responsibilities

• Support clients based on individual goals and ensure physical, mental, emotional, and social needs are met.
• Provide support and training per the service plan; assist with hygiene, bathing, toileting, escort, mealtime, and transitions.
• Teach self-control and self-management skills per training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Help clients develop and follow a daily activities schedule.
• Assist with arranging appointments and transportation and ensure proper documentation.
• Promote safety and follow all safety procedures.
