Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) to provide hands-on, person-centered care to individuals with IDD in home and community settings in Pennsylvania.

The role emphasizes support for daily living activities, behavior management, documentation, and community inclusion, with flexible scheduling and field-based work. A high school diploma and English proficiency are required, with training and HIPAA-compliant documentation expected.

Qualifications

• High school diploma or equivalent required.
• Prior caregiving, home health, or DSP experience preferred.
• Ability to use EVV and electronic documentation.
• English proficiency required; bilingual a plus.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health and safety.
• Maintain structured routines and document accurately.
• Support transportation to appointments and community outings.
• Participate in trainings and team meetings.
