Qualifications

• High school diploma or equivalent.
• English proficiency required (spoken and written).
• Experience with electronic documentation is preferred.

Responsabilités

• Provide hands-on, person-centered support per care plan.
• Assist with personal care, daily living activities, meals, mobility, and medication support as authorized.
• Maintain documentation and participate in trainings and team meetings.
• Support transportation and community participation, including scheduling and outings.
