Job summary

OneWell Health Care in King of Prussia, PA is seeking a dedicated Caregiver / Direct Support Professional (DSP) to support individuals with IDD and autism in home and community settings. This field-based role emphasizes respectful, person-centered care and helps clients with daily living activities, health monitoring, and community inclusion.

The DSP will implement ISP/IPP plans under supervision, maintain accurate documentation, and participate in trainings.

Qualifications

• High school diploma or equivalent.
• Preferred: prior caregiving experience with IDD/IDD.
• Experience in home health or direct support settings.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized; implement behavior support strategies.
• Maintain accurate documentation, participate in trainings, and support transportation and community participation.
