Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will implement individualized service plans, assist with daily living activities, and help promote independence and participation in community activities.

Responsibilities include documentation, trainings, and collaboration with an interdisciplinary team.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving or direct support preferred.
• English proficiency required (spoken and written).
• Driver's license may be required depending on assignment.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety and well-being.
• Maintain documentation, attend trainings, and participate in team meetings.
