Job summary

00 RHA Health Services, LLC is seeking a Direct Support Mentor to guide and train Direct Support Employees, ensuring coverage across three shifts and advocating for residents with IDD and mental health needs. The role may involve occasional overnight stays when schedules require it.

The position emphasizes mentorship, documentation, and collaboration with the Residential Team Leader to deliver person-centered supports aligned with each individual's plan and goals.

Qualifications

• Must have a high school diploma or GED.
• Prefer experience working with IDD individuals.
• At least one year of experience working with the population served.
• Must have a valid driver s license (this is a condition of employment) including personal vehicle insurance coverage.
• Must maintain proficiency in company sponsored training and certifications and privileging as required.
• Maintain CPR and First Aid certification , Med Tech and NCI certifications and other core competency training that may be specified and/or required by future company regulations or service definitions of service being provided
• Depending on service being provided must be clinically supervised by a qualified professional according to supervision requirements as specified by licensure or certification requirements of appropriate discipline or service being provided.
• Must be able to demonstrate proficiency in CPR from floor level.

Responsibilities

• Assist people supported to identify personal outcomes best suited to their interests and desires.
• Identify supports needed for personal outcomes with the Interdisciplinary Team.
• Promote goals, dignity, respect, and quality of life for people supported.
• Provide opportunities for choice and empower individuals to exercise control.
• Report all incidents of abuse, neglect or exploitation immediately.
• Help maintain cleanliness of group homes and monitor maintenance needs.
• Support people in activities of daily living and routine tasks.
• Assist in preparing food and cleaning residences and vocational spaces.
• Help people identify valued social roles in community settings.
