Job summary

Fresenius Medical Care is seeking a Registered Nurse CAP 2 for our Outpatient program. The RN CAP 2 will coordinate and provide nephrology care, including assessment, planning, intervention, and evaluation for a group of patients undergoing hemodialysis.

The role involves collaborative patient education and guidance to nursing staff and may include precepting new employees. The RN CAP 2 participates in quality improvement activities, ensures adherence to policy and regulatory standards, and

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state RN licensure.
• Current CPR/BLS certification.
• Must meet state practice requirements.

Responsibilities

• Perform patient data collection and analysis during hemodialysis sessions and adjust care plans as needed.
• Educate patient and family about learning needs and ongoing care.
• Provide safe delivery of hemodialysis therapy in collaboration with the care team.
• Administer medications per prescribed protocols and document effectiveness.
• Respond to emergencies and participate in quality improvement activities.
• Serve as a resource and mentor to new staff and support CAP requirements.
