Job summary

Fresenius Medical Care is seeking an Outpatient RN CAP 2 to provide comprehensive nursing care for an assigned group of patients undergoing hemodialysis. The role includes assessment, planning, intervention, and evaluation, with collaboration across RNs, LVN/LPNs, and patient care technicians.

The position emphasizes patient education, leadership, and quality improvement activities within the End Stage Kidney Disease care team.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification
• Must meet practice requirements in the state in which he or she is employed.

Responsibilities

• Perform nursing assessment, planning, intervention, and evaluation for an assigned group of patients during outpatient hemodialysis.
• Document data in the patient medical record and adjust treatment plans as indicated; notify supervisor as needed.
• Educate patients and families about learning needs and provide ongoing education.
• Administer medications as prescribed or per approved algorithms; document justification and effectiveness.
• Direct and collaborate with direct and ancillary staff to ensure safe and effective delivery of hemodialysis therapy.
• Lead and mentor staff; serve as a resource and role model for new and incumbent employees.
• Coordinate correct laboratory collection, processing, and shipping procedures.
