Job summary

Fresenius Medical Care in Fort Wayne, IN is seeking an entry-level Registered Nurse for the Outpatient CAP 1 program. You will provide and coordinate patient care, including assessment, planning, intervention and evaluation for dialysis patients.

As part of the End Stage Kidney Disease care team, you will participate in teaching, leadership functions and quality improvement activities to enhance patient outcomes and facility operations.

Qualifications

• Entry level RN with less than 2 years in nephrology nursing.
• Hemodialysis experience (preferred).
• Graduate of an accredited School of Nursing; RN licensure; CPR/BLS valid.

Responsibilities

• Perform assessments, planning, intervention and evaluation for assigned patients.
• Document data collection and treatment plans in medical records.
• Educate patients and families about care and treatment options.
• Deliver safe hemodialysis therapy under supervision and guidance of the team.
• Administer medications as prescribed or per protocol and document effects.
• Assist with emergency responses as needed.
• Collaborate with Interdisciplinary Team to update care plans.
• Lead or support daily operations with patient care staff as assigned.
• Ensure proper lab collection, processing and shipping procedures.
• Support training and orientation of new staff as assigned.
