Job summary

The Washington State Department of Social and Health Services, Developmental Disabilities Administration in Yakima, WA, seeks an experienced Resource Case Manager to deliver specialized case management and resource assistance to individuals with developmental disabilities and their families.

In this role you will conduct assessments, plan services, coordinate with state agencies, and ensure person-centered practices.

Qualifications

• Bachelor's degree or equivalent in social sciences or allied field is required.
• Progressive case management experience with individuals with developmental disabilities preferred.
• Demonstrated ability to plan, coordinate, and document services with person-centered focus.

Responsibilities

• Identify client support needs, plan, and authorize payment for services.
• Schedule and conduct home visits in clients' residences with families.
• Facilitate person-centered goal planning and update assessments and service plans.
• Complete and revise client assessments within established timelines.
• Identify clients in crisis and refer to appropriate authorities or services.
• Determine and evaluate eligibility for enrolled programs.
• Review and monitor client plans, reports, and progress.
• Obtain translation and interpretation services when needed.
• Maintain complete case files and payment records.
• Collaborate with community partners and state agencies.
• Represent DSHS professionally at meetings.
