Job summary

Road to Responsibility in Plymouth, MA is seeking a full-time Support Specialist to assist adults with developmental disabilities in a day habilitation setting. The role involves coordinating therapies, maintaining records, and supporting individuals with daily living and behavioral plans.

The position requires case management experience, a valid driver's license, and proficiency with MS Word. A CORI background check and TB screening are required; training will be provided as needed.

Qualifications

• Must have case management experience in a health care setting.

Responsibilities

• Review treatment and habilitation goals monthly and inform staff of changes.
• Supervise ISP/DHSP implementation and maintain records.
• Coordinate OT/ PT/ Speech/ Nursing and behavioral services for caseload.
• Prepare monthly progress notes, two-year Day Habilitation Service Plans, assessments and objectives.
• Provide transportation to community activities as needed.
• Adhere to RTR policies and safety standards.
