Job summary

Lifeworks, Inc. is hiring a full-time Case Manager in Massachusetts to support adults with ID/DD. You will provide individualized instruction, coordinate services with the interdisciplinary team, and develop Day Habilitation Service Plans while maintaining progress notes and documentation.

The role requires a related degree with experience (or HS diploma with extensive experience) and a valid driver's license. Strong communication, teamwork, and computer skills are essential for success.

Qualifications

• Experience working with individuals with ID/DD.
• Bachelor's or Associate degree in a related field with 2 years of experience, or HS diploma with 5 years of related work experience.
• Pass fingerprint background check, CORI, DPPC and driving background checks; valid driver's license.
• MAP certification preferred.

Responsibilities

• Provide individual and small group instruction in self-care, ADLs, communication, social and motor skills.
• Coordinate services with Interdisciplinary Team.
• Develop Day Habilitation Service Plans (DHSP) and maintain progress notes.
• Write bi-annual and annual summaries and maintain participant records.
• Attend supervision sessions, staff meetings, and program development discussions.
