Job summary

Road To Responsibility Inc. is seeking a full-time case manager to support adults with developmental disabilities in a day habilitation setting.

You will work as part of an interdisciplinary team coordinating OT, PT, Speech, Nursing, and behavioral services, while documenting progress and monitoring plans. Responsibilities include monthly goal review, development of service plans, and maintenance of records.

Qualifications

• Experience with case management and case reviews in a health care setting.
• Willingness to authorize a Criminal History Background Check (CORI).
• Valid Driver’s License and ability to travel as needed.

Responsibilities

• Review treatment goals monthly and inform staff of changes.
• Coordinate therapy services for assigned individuals.
• Prepare reports and progress notes per DDS and agency requirements.
• Attend mandated trainings and maintain required records.
• Provide transportation to community activities as needed.
