Job summary

Road to Responsibility in Plymouth, Massachusetts, is seeking a dedicated professional to support and evaluate adults with developmental disabilities in a day habilitation setting. You will work as part of an interdisciplinary team to coordinate therapeutic services and promote person-centered care.

The role requires strong documentation, collaboration with families, and adherence to safety and behavioral plans.

Qualifications

• Experience with case management and case reviews in a healthcare setting.
• Valid Driver’s License.
• Completion of all state and agency mandated trainings.
• Willing to authorize a Criminal History Background Check (CORI).
• Tuberculosis screening within the previous 12 months.

Responsibilities

• Review each recipient’s treatment and habilitation goals monthly and inform staff of changes.
• Supervise the ISP/DHSP implementation and maintain records.
• Coordinate OT, PT, Speech, Nursing and behavioral services for the caseload.
• Prepare reports in accordance with state and agency requirements.
• Provide transportation to community activities as needed.
• Attend Safety Care training and implement behavioral plans as assigned.
• Interact with co-workers, clients, families, and funders professionally.
