Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist to deliver in-home care to patients in the Detroit area. This field-based, full-time role expects a patient-centered approach, strong clinical judgment, and proactive coordination with RNs, physicians, and caregivers.

You will manage a caseload, perform comprehensive in-home assessments, develop and adjust plans of care, document promptly, and drive outcomes to reduce avoidable hospitalizations.

Qualifications

• Active Michigan PT license in good standing.
• Strong clinical judgment and field independence.
• Clear communication and thorough documentation.
• Reliable follow-through on orders and patient goals.
• Daily travel across the assigned territory with a valid license.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute plan of care: gait/balance, transfers, strengthening, mobility.
• Set measurable goals and adjust treatment as patients progress.
• Drive multidisciplinary coordination with care team and physicians.
• Manage treatment and PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly, defensibly, and consistently.
