Job summary

Adaptive Home Health is seeking a Physical Therapist for field-based work in Detroit, MI. You will deliver skilled PT visits, manage a full caseload, and coordinate with a multidisciplinary team to improve patient outcomes.

Requires active Michigan PT license, strong clinical judgment, and ability to travel daily. Compensation ranges from $100,000 to $130,000 yearly with productivity-based incentives and benefits for full-time staff.

Qualifications

• Active Michigan PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and thorough documentation.
• Comfort with daily travel across assigned territory.

Responsibilities

• Own the caseload as the primary PT in patients' homes.
• Complete in-home assessments and deliver skilled PT visits.
• Develop and execute a clear plan of care for mobility, transfers, and safety.
• Document promptly and accurately in the EMR.
• Coordinate with care team to adjust treatment as needed.
