Job summary

Adaptive Home Health, LLC in Detroit is hiring licensed PTA clinicians for field-based visits under a supervising PT. The role focuses on functional mobility, gait, balance, and home safety while leveraging AI-enabled documentation tools to reduce after-hours charting.

You will deliver high-quality home health care, manage a full caseload, and collaborate with a multidisciplinary team to drive outcomes and patient satisfaction.

Qualifications

• Active Michigan PTA license in good standing.
• Reliable transportation, valid driver's license, auto insurance.
• Home health experience strongly preferred.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope.
• Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations.
• Strengthening, functional mobility, and home safety assessment/modification education.
• Patient/caregiver coaching with practical home programs.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on functional mobility and measurable progress.
• Set/track objective progress within PTA scope and communicate updates to supervising PT.
• Drive multidisciplinary coordination and keep the care team moving.
• Coach patients and caregivers with practical home programs that actually get done.
• Document promptly and thoroughly — defensible, efficient, and consistent.
