Job summary

Adaptive Home Health, LLC seeks a Physical Therapy Assistant to deliver skilled PTA visits in Detroit-area homes with supervision from a PT. The role focuses on functional mobility, gait, balance and safety, using our AI-native platform to streamline charting.

Qualifications

• Active Michigan PTA license (in good standing) is required.
• Reliable transportation and valid driver's license.
• Home health experience strongly preferred.
• Ability to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, and home safety.
• Set/track objective progress and communicate updates to the supervising PT.
• Drive multidisciplinary coordination to keep the care team moving.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly — defensible and efficient.
