Job summary

Adaptive Home Health is seeking a Physical Therapist in Detroit to deliver excellent home health care while building our clinical standard and patient trust. You will own a full caseload, perform in-home assessments, and guide a plan of care focused on gait, transfers, and functional mobility.

You will coordinate with the care team, supervise PTAs as needed, and coach patients and families. A Michigan license and daily travel are required, with a full-time schedule.

Qualifications

• Active Michigan PT license in good standing.
• Strong clinical judgment; comfortable working independently in the field.
• Able to escalate changes to RN/MD with clear rationale.
• Strong communication and documentation.
• Reliable follow-through on orders and patient goals.
• Current driver’s license; comfortable with daily travel.
• Able to carry/transport basic therapy equipment.
• Competent in core home health PT scope: gait/balance, transfers, strengthening.
• Patient/caregiver coaching with practical home programs.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, mobility, safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Drive multidisciplinary coordination and keep care team moving.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly; maintain defensible records.
