Adaptive Home Health, LLC is seeking a dedicated Physical Therapy Assistant (PTA) to deliver skilled visits in patients' homes across Detroit and surrounding communities. This full-time, field-based role offers flexible scheduling to fit your needs and a support-driven environment with productivity targets.

You will work under a supervising PT, leverage an AI-native platform to streamline charting, and focus on gait training, transfers, balance, and home safety.

Qualificações

• Active Michigan PTA license (in good standing)
• Reliable transportation, valid driver's license, auto insurance
• Home health experience strongly preferred
• Able to carry/transport basic therapy equipment
• Competency in core home health PTA scope

Responsabilidades

• Deliver skilled PTA visits in the home under the supervising PT's plan of care
• Execute treatment plans focusing on gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention
• Set/track objective progress within PTA scope and communicate updates to supervising PT
• Drive multidisciplinary coordination and keep the care team moving
• Coach patients and caregivers with practical home programs
• Document promptly and thoroughly — defensible, efficient, and consistent
