Job summary

Adaptive Home Health seeks a Physical Therapy Assistant (PTA) to deliver skilled home visits in the Detroit area, working under a supervising PT and leveraging our AI-native platform to streamline charting and scheduling.

You will manage a full caseload, focus on functional mobility, and collaborate with a multidisciplinary team to achieve high-quality patient outcomes. This is a full-time role with flexible scheduling options and a productivity-based compensation model.

Qualifications

• Active Michigan PTA license in good standing.
• Reliable transportation with valid driver's license and auto insurance.
• Home health experience strongly preferred.
• Ability to carry/transport basic therapy equipment.
• Competent in core PTA home health scope including gait, transfers, balance, strengthening.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT's plan of care.
• Execute treatment plans focused on functional mobility and safety.
• Set and track objective progress and report to supervising PT.
• Drive multidisciplinary coordination and keep the care team aligned.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly.
