Job summary

Adaptive Home Health in Detroit is seeking a dedicated Physical Therapist to deliver in-home PT care across our field-based team. You’ll manage a full caseload, drive outcomes, and partner with physicians and RNs to coordinate care.

Responsibilities include in-home assessments, gait and balance training, transfers, strengthening, home safety, and caregiver coaching. A Michigan PT license and daily travel are required.

Qualifications

• Active Michigan PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and documentation skills.
• Reliable follow-through on orders, frequencies, and goals.
• Willingness to travel daily across assigned territory.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, mobility, safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Drive multidisciplinary coordination: communicate changes, align orders, and keep care team moving.
• Manage treatment/outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly — defensible and efficient.
