Job summary

Endeavors in Saint Paul, MN is seeking a Disaster Human Services Case Manager to provide survivor-centered case management during field or remote deployments. You will conduct intake and screening, assess unmet needs, develop recovery plans, coordinate resources, and maintain documentation, while protecting privacy and ensuring equitable access.

Qualifications

• Bachelor's degree in a related field is preferred; equivalent education/experience may be considered.
• Relevant experience in case management, human services, crisis intervention, benefits/resource navigation, disaster recovery, or community-based services preferred.
• Professional language capability beyond English is desirable; deployments may require additional languages.
• Must complete required screening, credentialing, cybersecurity/privacy training, and disaster management training before deployment.

Responsibilities

• Conduct disaster survivor intake, screening, and needs assessments using approved tools and procedures.
• Develop individualized recovery plans with survivors, set goals, identify barriers, and update plans as needed.
• Identify, verify, and coordinate available federal, state, tribal, local, nonprofit, and community resources.
• Provide referrals and follow-up; help survivors navigate benefits and recovery resources.
• Maintain survivor records, progress notes, referrals, and required statistics in approved systems.
• Protect PII and comply with privacy, confidentiality, and cybersecurity requirements.
• Deploy within required timeframes and work in field, remote, or austere environments as assigned.
• Demonstrate exceptional customer service, prioritizing the needs of children, families, veterans, or clients.
