Job summary

State of Maryland - MDH Local Health - Dorchester County seeks a Development Disabilities Targeted Case Management Coordinator of Community Services to guide individuals through eligibility for services, develop and monitor PCPs, and coordinate assessments as needed. The role ensures timely documentation and eligibility determinations with DDA.

The CCS coordinates entry into the system and supports clients with plan development and service monitoring, following COMAR requirements and department

Qualifications

• Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
• Experience: One year of administrative staff or professional work in a health care setting.
• Notes: 1) 30 college credit hours may substitute for the required experience. 2) Military service experience in health-related fields may substitute for experience/education.

Responsibilities

• Assist individuals with applying and gathering documentation for DDA eligibility.
• Facilitate development of the Person-Centered Plan (PCP) and monitor its services.
• Complete additional assessments as determined by the client's situation.
