The Development Disabilities (DD) Targeted Case Management (TCM) Coordinator of Community Services (CCS) provides a fixed point of entry into the service delivery system for individuals with developmental disabilities. This position will assist the individual in the preparation of the application and gathering of necessary documentation for DDA to make an eligibility determination. Further, the CCS will facilitate the development of the Person-Centered Plan (PCP) and monitor the PCP and its services. When required, additional assessments are completed as determined by the client's individual situation.
