This Position Will Assess And Determine Eligibility For DDA Services And Provide Referrals For Those Services. This Position Will Arrange Access To Day, Residential And Other Support Services. This Position Will Facilitate Annual Planning Meetings And Assist Individuals And Their Families With Developing a Plan Of Service. This Position Will Develop The Person-Centered Plan (PCP) And Conduct Periodic Monitoring Of The PCP And Satisfaction With Delivered Services. This Position Will Follow Individuals On The Waiting List Registry And Assist With Transition Planning. This Position Will Maintain Accurate Documentation Of All Services Provided. These Efforts Will Support Individuals In Maintaining Community Living, Promoting Individual Choice And Improving Overall Quality Of Life. Education : Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience : One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
The desired candidate should possess the following: 1. Experience working with individuals who have intellectual or developmental disabilities. 2. Experience with case management.
