Opportunity Builders in Maryland is seeking a case manager to coordinate person-centered services for individuals with disabilities. The role focuses on advocacy, service planning, and collaboration with CCSs and families within the PCP framework.

The position requires experience in disability services, strong communication, and meticulous record-keeping to support DDA funding and LTSS compliance.

Qualifications

• Minimum 1 year of experience coordinating services for individuals with disabilities.
• High school diploma or equivalent; bachelor's degree preferred.
• Strong written and oral communication skills.
• Highly organized with ability to work independently and in a team.
• Experience using a client management system, iCare Manager preferred.
• Familiarity with LTSS and DDA/DORS regulations.

Responsabilités

• Provide case coordination and advocacy for participants in OBIs programs.
• Coordinate PCP meetings with CCSs and participants' teams.
• Develop Service Implementation Plans and PCP goals.
• Ensure accurate documentation and funding requests to DDA.
• Maintain participant records and ensure regulatory compliance.
• Assist with transportation and direct care as needed.
• Communicate with families and CCSs regularly.
