Qualifications

• Strong customer service skills and ability to manage multiple projects.
• Ability to lead interdisciplinary rounds and coordinate care.

Responsibilities

• Perform high-level assessment and reassessment to set care priorities.
• Plan, organize, and implement goal-directed patient care.
• Coordinate with interdisciplinary team to ensure safe, cost-effective care.
• Advocate for patient self-determination and choice.
• Ensure accurate documentation reflecting illness severity.
